Patients discharged after treatment do not stop needing care. The days, weeks, and months after leaving hospital are often the most medically uncertain. Sevanun's remote care programme changes that reality — enrolling patients into a structured care ecosystem where their health is tracked continuously, a dedicated team monitors every signal, and clinical support is never more than a call away. The result: patients feel safe, caregivers feel confident, and clinicians have the data they need to act early.

The gap between discharge and the next appointment is where most care breaks down.


Patients with complex conditions — cancer, chronic illness, post-surgery recovery — face a difficult transition home. Without structured monitoring, symptoms worsen silently. Caregivers feel helpless. Families based abroad cannot see what is happening. And by the time the next appointment arrives, a manageable issue has become a hospital readmission. The emotional toll compounds the physical: seeing other patients in hospital surroundings — in various stages of illness — is itself a source of anxiety that patients repeatedly described as demotivating.

It is very convenient — consulting at the comfort of home in the middle of all household work. No travel, no waiting. And seeing patients with various stages of diseases in hospital surroundings brings down the confidence. This is totally avoided.
